3. Fill in or print the registration form and admission ticket completely, accurately and clearly, one registration form and admission ticket per person.
4. Three recent bareheaded color photos of the candidate (one each for registration form, admission ticket, and "Social and Art Level Examination Certificate"). Write the candidate's name, declared major, and declared level on the back of the photo.
5. Candidates who have obtained music grading certificates from other grading institutions can apply for the same level as required by the Art Development Center of the Ministry of Culture and Tourism.
