The so-called direction means major. There are three most common majors in musicology. This is a relatively large and general division. If you are studying piano, you should choose the piano direction. If you are studying vocal music, you should choose the vocal music direction first. In fact, piano also belongs to the instrumental music direction, but piano is listed separately because there are more enrollment and applicants.
