Documents and detailed instructions required for maternity insurance reimbursement in Shenzhen:

1, reimbursed social security card+reimbursed ID card+original ID card of the agent; Copy (requirement: reimbursement applicant's hospitalization invoice+social security card+ID card is copied on a piece of A4 paper, and paper content typesetting is not required)

2. The original invoice of the reimbursement person's hospitalization; Copy (requirement: reimbursement applicant's hospitalization invoice+social security card+ID card is copied on a piece of A4 paper, and paper content typesetting is not required)

3. The original detailed list of hospitalization expenses of the reimbursement person (required to be stamped by the hospital)

4. The original proof of the work unit of the reimbursement person;

5. Original disease diagnosis certificate of the reimbursement person (to be handled together with Article 7)

6, maternal and child health handbook original reimbursement;

7. Copy of the reimbursement person's hospitalization medical records (including admission records, doctor's advice, operation records, discharge records and related inspection reports, which are copied in the hospital medical record room and stamped with the official seal of the hospital).

8. The original of the applicant's household registration book, the original of the applicant's marriage certificate, the original of the applicant's family planning service certificate and the original of the baby's birth medical certificate; Copy is required, and paper content typesetting is not required.

9 original bankbooks of the four major state-owned banks (China, agriculture, industry and construction) opened by the payer in Shenzhen; Photocopy requirements: the passbook of the reimburser+ID card of the reimburser+ID card of the handler should be copied on 1 A4 paper, and the content and layout are not required. Need to make 2 copies, 65,438+0 for window auditors and 65,438+0 for office financial accountants.

Note: Employees participating in maternity medical insurance who are hospitalized outside the city for prenatal examination, delivery or family planning operation must submit the following documents to the medical insurance department of their social insurance institutions for reimbursement within 6 months from the date of discharge.
