Article 9 The basic conditions that a chain of old-age care services should meet are as follows:

(1) All member units are united on a voluntary basis;

(2) The manager of the chain organization is a registered apartment for the elderly;

(3) The manager of the chain organization has the necessary funds, and the funds are not less than 2, yuan/bed according to the number of beds in the chain organization;

(4) the managers of chain institutions have strong management ability and complete chain institutions management system;

(5) unified name, unified logo, unified management mode and service specification;

(6) The building area of the service place of each member unit is more than 1 square meters, and the number of beds set is no more than 2;

(7) The requirements of service places, fire control and sanitation of each member unit shall refer to the provisions of Article 8 of these Measures.

Article 14 When applying for organizing an apartment for the elderly, the applicant shall submit the following materials:

(1) an application and a feasibility study report;

(2) the qualification certification materials of the applicant;

(3) certification materials on the source of funds for the proposed apartment for the elderly;

(4) the certification materials of the place to use the apartment for the elderly;

(5) Other materials required.

If the planned apartment for the elderly meets the opening conditions, it shall apply to the district/county (city) civil affairs department for the Certificate of Approval for the Establishment of Social Welfare Institutions, and submit the following materials:

(1) an application;

(2) Approval Form of Social Welfare Institutions in Heilongjiang Province;

(3) certificate of ownership of the service place or lease contract;

(4) Articles of association and rules and regulations;

(5) a list of managers, professional technicians and service nurses, a copy of valid certificates and a certificate of health status of the service nurses;

(6) If medical care, rehabilitation training and other services are offered, and this apartment for the elderly has no medical practice qualification, it is necessary to provide a long-term cooperation agreement with medical institutions;

(7) Other materials required.
